The CPSP clinical pastoral education/training application may be downloaded and submitted via email or postal mail. Completed applications should be submitted to the CPSP CPE/Training center that you are applying to attend.

Certified Individuals and Chapters are encouraged to provide educational events for clergy, hospitals and the community. Programs that are reviewed by a Chapter can receive Continuing Education Credits for participants who complete the event. Download this form to complete when requesting CEU�s for your sponsored event(s).
